Concierge Care Management & Support for Older Adults

During the initial assessment, I will visit you in your home and learn more about your health, home, basic finances, and how your world works. Initial assessments are $175 and last between 1.5 hours and 2 hours.

Some things I might ask about include; 

• End-of-life planning documents such as a living will, power of attorney or a health care proxy.
• Who supports you in your family?
• Do you or someone else handle your bills?
• How do you get groceries, to medical appointments, etc.?
• Who supports you emotionally?
• What makes life difficult right now?

From there, I will create a plan built for you with suggestions on resources that you could look into and ways I could support you. Think of it as an a la carte menu of resources and support. You simply pick what you want and leave what you don't. 

Aging can be an incredibly difficult time, but my hope is that I can be a support and a listening ear to help you through it. My goal is to honor where you are at, provide empathy, understanding and help you get to a place where you feel better.

Being a caregiver for your loved one is incredibly rewarding, but it can bring up many emotions. The road is long and often has many bumps and unforeseen detours, whether you live locally or afar. Caregiving can become lonely and at times, frustrating due to the many systems you are trying to navigate.

My social work experience includes being an; inpatient hospital social worker, discharge planner, emergency department social worker and a home health and hospice social worker, which allows me to provide you with the information needed to make the best decision for your loved one. I can also help navigate transferring to a higher level of care and help provide transitional care from the hospital to short-term rehab or back home with supports, if needed.

Often times, older adults are eligible for services they are unaware of and/or they do not know the resources in their area that are available to them. I can help guide you to connect with resources tailored to your specific needs.

I can also help provide education to individuals and their families in preparation of completing end-of-life planning. To ensure that your end-of-life wishes like your code status and limitations to treatment, are well documented with your medical team, health care proxy and family.
